Cancellation Policy
There is no charge for a cancelled office visit when at least a 24-hour notice is given. Otherwise, full payment for the missed appointment is required, unless due to a medical emergency.
Insurance and Payment Information
Full payment is due at the beginning of each visit. I am not accepting insurance. Verify your health insurance coverage prior to our first visit.
Statements (superbill) to obtain reimbursement from your health insurance company will be provided.

Initial 60-minute consultation: $300.00 Sliding Scale $240.00
45-minute visit: $225.00 Sliding Scale $145.00
25-minute visit: $150.00 Sliding Scale $115.00
